Subscription Terms
Access to the Service Form Pro system is offered by Flourish Growth Solutions, LLC. Your subscription begins as soon as your initial payment is processed. Your subscription will automatically renew each month without notice until you cancel. By proceeding, you agree to store your payment method through a secure third party (Stripe) and to automatically charge your payment method(s) every month until you cancel.
We may change your plan’s rate each monthly renewal term, and we will notify you of any rate change with the option to cancel. If the applicable included tax or duty changes during your one-month term, we will accordingly adjust the tax-inclusive price for your plan on your next billing date.
If your primary payment method fails, you authorize us to charge any other payment method in your account. If you have not provided us a backup payment method(s) and you fail to provide payment, or if all payment methods in your account fail, we may suspend your subscription and your access to the system will be limited. It is your responsibility to keep your payment method up to date with the third party payment processor (Stripe).
Ownership of Data
While you will have access to download all previous service reports, all setup data contained within the Service Form Pro system including users, property records, and service reports data will be considered property of Flourish Growth Solutions, LLC DBA Service Form Pro and retained in the system for a period of 6 months from cancellation before deletion. Service Form Pro will take all best efforts to secure the data but will be under no obligation to provide you any of this data beyond the current billing cycle without an active subscription.
Cancellation Terms
You can cancel your subscription anytime via your account page or by contacting sales@serviceformpro.com. Your access to the Service Pro System will continue through the current billing period which in some cases may be up to 30 days. During this period it is your responsibility to download your past service reports you wish to archive. As no pro-rated credit will be issued for unused access during the period between cancellation and the end of the regular billing period, it is your responsibility to cancel before your payment is automatically charged via the third party processor (Stripe).
